Bipolar Disorder Might Be Overdiagnosed

WEDNESDAY, May 7 (HealthDay News) -- Bipolar disorder may be overdiagnosed, say researchers at Rhode Island Hospital and Brown University.

They found that fewer than half of patients previously diagnosed with bipolar disorder were assessed using a comprehensive psychiatric diagnostic interview -- the Structured Clinical Interview for DSM-IV (SCID).

Recent reports suggest that under-diagnosis of bipolar disorder may be an issue, while this study indicates that there's an equal or greater problem with overdiagnosis, the study authors said.

They looked at 700 psychiatric patients who were interviewed using the SCID and completed a self-administered questionnaire between May 2001 and March 2005. The questionnaire asked patients if they'd been previously diagnosed with bipolar or manic-depressive disorder by a health-care professional.

While 145 of the patients said they'd been previously diagnosed with bipolar disorder, only 43.4 percent were diagnosed based on the SCID. The study also found that people diagnosed based on the SCID were much more likely to have first-degree relatives with the disorder.

The study was published online inThe Journal of Clinical Psychiatryand was expected to be presented Wednesday at the American Psychiatric Association annual meeting, in Washington, D.C.

Overdiagnosis of bipolar disorder may lead to unnecessary use of medications and the risk of harmful side effects, noted lead author Dr. Mark Zimmerman, director of outpatient psychiatry at Rhode Island Hospital and an associate professor of psychiatry and human behavior at Brown University.

"Clinicians are inclined to diagnose disorders that they feel more comfortable treating. We hypothesize that the increased availability of medications that have been approved for the treatment of bipolar disorder might be influencing clinicians who are unsure whether or not a patient has bipolar disorder or borderline personality disorder to err on the side of diagnosing the disorder that is medication responsive," Zimmerman said in a prepared statement.

"This bias is reinforced by the marketing message of pharmaceutical companies to physicians, which has emphasized the literature on the delayed and under-recognition of bipolar disorder, and may be sensitizing clinicians to avoid missing the diagnosis of bipolar disorder."

Zimmerman concluded: "The results of this study suggest that bipolar disorder is being overdiagnosed, and we recommend that clinicians use a standardized, validated method in diagnosing bipolar disorder."

Crohn's disease, school district's decision keeps Orange student from graduation

PEPPER PIKE -- Crohn's disease makes even typing a few words difficult for 18-year-old Josh Shapiro.

"I have pains in my stomach," the Orange High School student said. "Sometimes it feels a little difficult to breathe. I have constant joint pain."

The fatigue associated with the disease made Shapiro and his parents decide to reduce his class schedule from five to three classes.

"Because I take fewer classes and because I am at the school less often, the class of 2008, half of them don't know I am part of the class of 2008."

He still maintains an A-average but Shapiro's three credits short of graduating.

"It's very healthy to be part of the crowd sometimes and for Josh, that's all he wants," his father, Jerry Isaak- Shaprio, said. "For me, that's very frustrating."

Josh's mother, Deborah Isaak- Shapiro added, "We're not asking he get a diploma, we're asking he participate in graduation with his class."

Despite several phone calls, emails and even a meeting with the Orange Board of Education, the answer is still no.

The school district emailed a statement to Channel 3 News on Wednesday. It said, "The Orange School District policy clearly states the requirements for graduating and participating in commencement exercises. When a student does not meet those conditions, he or she cannot partake in commencement."

"It certainly makes me a little angry, frustrated," Josh Shaprio said, "kind of sad."

If Josh felt like an outcast before, he doesn't now. One student sensitive to his struggle started a Facebook page called "let josh graduate!" There are 180 students in his class and so far, 75 of them signed on as supporters.

The graduation ceremony is scheduled for Sunday.

Shapiro says even if the district is doesn't make an exception, perhaps his story will make a difference for the next student with a similar problem.

Researchers Identify Determinants of COPD Exacerbation Frequency: Presented at ATS

TORONTO -- May 26, 2008 -- Chronic intermittent pulsed therapy with moxifloxacin provided significant reductions in the frequency of exacerbations in chronic obstructive pulmonary disease (COPD), according to findings presented here at the 104th International Conference of the American Thoracic Society (ATS).

The results were presented here on May 18 by lead author Sanjay Sethi, MD; Attending Physician; Department of Pulmonary, Critical Care, and Sleep Services; Veterans Affairs Western New York Healthcare System in Buffalo, New York.

Frequent exacerbations of COPD increase both morbidity and mortality. They may result in a faster decline of lung function and may lead to a significant reduction in quality of life. Reducing both the frequency and severity of these exacerbations is one of the main goals of maintenance therapy.

Long-acting bronchodilators and inhaled corticosteroids are commonly used but have only moderate efficacy.

In a poster presentation, Dr. Sethi said that as many as 40% to 50% of COPD exacerbations may be related to bacterial infections. Bacterial strains that are new to patients are thought to be the main cause of acute exacerbations. Current therapeutic approaches focus on treating the exacerbations as they happen.

An effective suppressive regimen might involve disrupting the cycle of chronic bacterial colonisation and chronic inflammation, which might lead to fewer exacerbations.

That rationale led to this randomised, double-blind, parallel-group, multicentre trial conducted at 76 centres in 15 countries. Treatment was administered for 5 days every 8 weeks for a total of 6 courses. Patients were randomised to moxifloxacin 400 mg once daily or placebo once daily.

Patients were evaluated for exacerbations and adverse events for up to 24 weeks post-treatment.

The primary efficacy endpoint was frequency of exacerbations from randomisation to the end of treatment at 48 weeks in the per-protocol (PP) end-of-therapy (EOT) population.

A total of 1,404 patients were assessed for eligibility; 573 patients were randomised to receive moxifloxacin and 584 to placebo. Four patients in each group did not receive treatment, thus the intention-to-treat (ITT) arms had 569 and 580 patients, respectively. The PP EOT populations consisted of 351 patients in the moxifloxacin group and 387 in the placebo arm.

Approximately 31% of PP EOT patients in both arms were colonised by a respiratory pathogen at baseline. Approximately 10% of both groups were colonised by Haemophilus influenzae at baseline.

In both the ITT and the PP EOT groups, the significant variables influencing the number of exacerbations (P <= .05) were the severity of the underlying COPD as measured by the percent of predicted forced expiratory volume, treatment versus placebo, number of exacerbations in the previous year (<2>=2), and baseline colonisation with H influenzae (no baseline colonisation vs colonisation).

Geographic regions also demonstrated significant differences. Each region was compared with the mean value for all the other regions. In the ITT population, the use of systemic corticosteroids was an additional risk factor for exacerbations.

The study results also confirmed that antibiotics such as the fluoroquinolone moxifloxacin may provide benefits to maintenance therapy by preventing exacerbations and the subsequent inflammatory response, the researchers noted.

Review suggests asthma drugs safe, effective in COPD

NEW YORK (Reuters Health) - Previous research has linked a class of asthma drugs known as long-acting beta agonists to increased risk of adverse respiratory-related events or death in patients with COPD (chronic obstructive pulmonary disease).

However, a report published this month indicates this may not be the case.

The new report suggests that long-acting beta agonists have beneficial effects in patients with moderate-to-severe COPD and do not appear to increase the risk of respiratory deaths. Examples of long-acting beta agonists used to treat asthma and other respiratory conditions include Advair, Serevent, Foradil and Symbicort.

In the study, Dr. Gustavo J. Rodrigo, from Hospital Central de las Fuerzas Armadas, Montevideo, Uruguay and colleagues pooled data from 27 randomized clinical trials that compared two long-acting beta agonists with placebo or with two long-acting "anticholinergic" asthma drugs (for example, Spiriva) in patients with moderate to severe COPD.

Compared with placebo, long-acting beta agonist therapy reduced severe COPD flare-ups by 21 percent and did not increase death from COPD.

Treatment with a long-acting beta agonist had benefits "in terms of lung function, quality of life, and use of daily rescue medication" with short-acting beta agonists, Rodrigo noted in comments to Reuters Health.

However, in an editorial published with the study in the journal Chest, Dr. Malcolm R. Sears from McMaster University, Hamilton, Ontario, Canada, urges caution in applying this finding to the real world. "In COPD trials, patients are carefully selected and monitored, and asthma is generally excluded. In real life, asthma and COPD can be hard to disentangle especially in older patients," he writes.

Hence, accepting the current results that a long-acting beta agonist alone is appropriate and safe in patients with COPD "may put some patients who in fact have asthma at risk if treated in this manner," Sears warns.

Rodrigo's team also found that treatment with a long-acting beta agonist combined with an inhaled steroid was associated with lower respiratory death compared with a long-acting beta agonist alone.

"Given the reduced mortality with the use of inhaled corticosteroids in combination with long-acting beta-2 agonist therapy, one would argue that, in COPD, concomitant inhaled corticosteroid therapy is preferable to long-acting beta-2 agonist monotherapy," Rodrigo told Reuters Health.

The pooled data also showed that treatment with Spiriva decreased the incidence of severe COPD flare-ups compared with a long-acting beta agonist.

Commenting on this finding, Sears writes: "Overall, daily tiotropium (Spiriva) appears to be foundational as a long-acting bronchodilator in COPD, despite some equivocation on this matter in current COPD treatment guidelines."
